Applicants are to submit Application Form A (Gansho)which are enclosed in Admission Information Book (Boshu-yoko), and also all of the required documents.
Please note that the downloaded Form A cannot be accepted.

Affix a 240-yen postal stamp to a self-addressed envelope (A4 size, Kakugata-nigou), write the name and address to which the envelope is to be sent, and then send the envelope to us.
We take care of both Graduate School of Engineering and Advanced Integration Science. Be sure to write the detailed information on the envelope as below.
1. Your name and address
2. Graduate School of Engineering
3. April 2012 Admission for master’s program
4. Admission Information Book (Boshu-yoko) and Application Forms (Gansho)
